As title says, weird that the loot did not spawn, are there any compensation and what is causing this? This is first time for me after hundreds of DM N runs

This is 2nd time happened in the same day. I'm not sure what is happening.

Please note the time stamp, tribute chest not spawning, there was no option for me to click "gimme the tribute loot" from the Orge NPC
